{"Key Responsibilities": "
  • Support participants with their individualized plans and goals, with a capacity building focus

  • Engage participants in the community, building connections and promoting social inclusion

  • Assist participants with daily needs, such as transportation and personal care

  • Work collaboratively with a small, cohesive team to achieve shared goals

  • Develop and maintain relationships with participants, their families, and other stakeholders

  • Contribute to the development of individualized plans and goals, tailored to each participant's needs

  • Participate in regular team meetings and contribute to the development of team goals and objectives

  • Ensure accurate and timely recording of participant progress and outcomes

  • Comply with all relevant policies, procedures, and legislation

  • Participate in ongoing training and professional development to enhance skills and knowledge

  • Perform other duties as reasonably required

  • Demonstrated experience working with people with a disability, either in a professional environment or through non-professional means or life experience

  • A sound understanding of disability and its effects on social relationships, independent living, daily needs, physical health, vocational, and employment needs

  • Demonstrated experience in supporting participants with individualized plans and goals, with a capacity building focus

  • Excellent communication, negotiation, and interpersonal skills

  • Ability to identify, organize, and utilize relevant resources effectively

  • Ability to function as a constructive and collaborative member of a small, cohesive team

  • Demonstrates qualities of empathy, compassion, and integrity

  • Current driver's license and own transportation

  • Ability to work in a fast-paced environment and adapt to changing priorities

  • Excellent time management and organizational skills

  • Ability to maintain confidentiality and handle sensitive information

  • Ability to work independently and as part of a team

  • Ability to prioritize tasks and manage multiple responsibilities
