About the Role

We are seeking a highly skilled Clinical Flow Coordinator to join our team at Central Adelaide Local Health Network. As a Clinical Flow Coordinator, you will play a critical role in managing inbound referrals of aged clients referred to Central Adelaide ACAT for a comprehensive assessment.

Key Responsibilities
  • Manage inbound referrals of aged clients using the National Screening and Assessment Form (NSAF) and the My Aged Care client record.
  • Enter accurate data from the intake/triage process into the My Aged Care system to determine appropriateness for assessment and clinical priority.
  • Provide up-to-date information on appropriate aged care pathways that clients may be eligible to access, including community, permanent residential, residential respite, and flexible care options.
  • Liaise with clients, carers, other health professionals, service providers, and internal and external stakeholders to ensure seamless care coordination.
  • Work within the Aged Care Assessment Programme guidelines and meet benchmarks and Key Performance Indicators (KPIs) set by the Commonwealth government and the Office for Ageing Well (OFAW) – SA Health.
