Job Role:

Care Advisor/Case Manager sought to oversee case management of Community Aged Care clients in Sydney's South East and Sutherland Shire.

About the Position:

This is a dynamic role that requires assessing, coordinating & reviewing client care needs within their budget.

Key Responsibilities:

  • Assessing client care needs and developing individualized plans.
  • Ensuring delivery of flexible and responsive services tailored to client needs and interests.
  • Maintaining accurate records and communicating effectively with clients, families, and healthcare professionals.
  • Developing and implementing care plans that meet unique needs and goals of each client.

Requirements:

  • Minimum 2 years experience in care planning and assessment or case management.
  • Highly developed planning, budgeting, time management, and negotiation skills.
  • Excellent communication and interpersonal skills for building strong relationships with clients, families, and healthcare professionals.
  • Ability to work independently and as part of a team with flexibility and adaptability.
