Job Title: Patient Care Coordinator
Job Description:The Patient Care Coordinator plays a vital role in the healthcare team, ensuring effective communication and coordination of patient care. This position requires strong organizational skills, attention to detail, and the ability to work effectively in a fast-paced environment.
Key Responsibilities:- Patient Record Maintenance: The Patient Care Coordinator is responsible for preparing, compiling, and maintaining accurate and up-to-date patient records.
- Communication Link: This role acts as a communication link within the patient-care area, handling phone calls, messages, and other communications.
- Patient Support: The Patient Care Coordinator supports the patient care team by performing multifunctional activities under the direction of Registered Nurses and assisting with tasks related to environmental, dietary, transportation, supplies, and light maintenance.
- Education: A high school level education with coursework in English grammar, composition, and basic mathematics is required.
- Experience: Six months of either direct patient care experience or clerical office experience, which has provided knowledge of utilizing a computer to access information, is necessary.
- Medical Terminology: Successful completion of a Medical Terminology course or equivalent and a score of 70% or greater on a screening medical terminology test are also required.
- Population Specific Competency: Ability to effectively interact with populations of patients/customers with an understanding of their needs for self-respect and dignity.
- Computer Skills: Knowledge of computer keyboard and comfort using Windows-based software systems.
- Teamwork: Ability to work well within a team environment by accepting and offering honest and constructive feedback, supporting team goals, and collaborating with patient care team members to consolidate work, solve problems, create new methods, and/or reduce costs.
- Heavy Exertion: Exerts 50-100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or ten to 20 pounds of force continuously to move objects.
- Exposure to Blood and Other Bodily Fluids: Risks of falls, strains, contagious diseases, etc., due to location within a healthcare environment.
- Competitive Pay: Salary range of $17.26-$20.71 per hour.
