We are seeking a dedicated Community Health Nurse to join our Medical Services team in a 0.4 FTE fixed term position until 30 June 2026.
This new role will support the delivery of chronic disease management (CDM), helping patients navigate long-term health concerns through high-quality care planning, health assessments, and follow-up support.
You will work closely with our GPs, allied health staff, and the broader care team to coordinate and deliver compassionate, person-centred care, supporting prevention, education, and patient empowerment.
Key Responsibilities:
- Support the delivery of CDM
- Conduct health assessments and care plans
- Provide follow-up support
Requirements:
- Registered Nurse with AHPRA registration
- Experience in community health or general practice (3+ years)
- Confident with CDM, care plans, and health assessments
- Skilled in immunisations, infection control and wound care
