Advanced Case Management Specialist

We are seeking experienced professionals to undertake rehabilitation assessment and case management services across various jurisdictions.
Key Responsibilities:- Conduct thorough assessments to identify clients' needs and develop effective case management plans.
- Collaborate with healthcare professionals, employers, and other stakeholders to ensure seamless service delivery.
- Provide high-quality interventions to promote optimal health outcomes and return-to-work rates.
- AHRPA accredited Physiotherapist or Occupational Therapist/ESSA approved Exercise Physiologist.
- Minimum 3 years of experience in occupational rehabilitation/RTW sector.
- Exceptional communication skills.
- Previous experience working with TAC preferred.
- Current driver's license and motor vehicle.
