About the Role

We are seeking an experienced and compassionate Care Coordinator to join our team at MiCare, a non-profit organisation dedicated to providing high-quality care services to individuals from diverse backgrounds.

This is a permanent full-time position based in Kilsyth, with two positions available.

Key Responsibilities:
  • Develop and implement individualised care plans that enhance the quality of life for our clients;
  • Liaise closely with internal and external stakeholders to ensure seamless delivery of care services;
  • Conduct initial assessments, ongoing reviews, and monitoring of client needs in accordance with MiCare's Home Care Policies and Commonwealth Government Aged Care guidelines;
  • Provide support and advocacy for clients and their carers as required;
  • Maintain accurate and detailed documentation of client interactions and progress;
  • Collaborate with relevant agencies and networks to promote MiCare's services;
  • Respond to phone and online enquiries, public education, and potential client outreach;
Requirements:
  • Demonstrated knowledge of sound case management within the aged care sector, preferably working with complex family and social issues;
  • Clear understanding of the Aged Care Quality Standards;
  • Solid understanding of Commonwealth Government community aged care programs (Commonwealth Home Support Program, Home Care Package program, and Veterans Home Care);
  • Proactive, reliable, with excellent communication skills and attention to detail;
  • Ability to work collaboratively as part of a multidisciplinary team;
  • Ability to meet compliance requirements, including current First Aid and CPR certificate, valid Victorian driver's licence with an insured and registered vehicle, and ability to provide evidence of current flu and COVID-19 vaccination;
Qualifications:
  • Tertiary qualification in Allied Health, Nursing, or Social Work;
Salary and Benefits:

We offer a competitive salary package ranging from $43.08 to $46.35 per hour, plus a $10,000 car allowance, 11.5% superannuation, and generous salary packaging benefits for the non-profit sector.
