We are currently seeking an experienced Healthcare Professional to join our team as a Service Coordinator in Kilsyth. This is a permanent full-time position, offering a competitive salary package and a hybrid work arrangement.

About the Role

The Service Coordinator plays a pivotal role in enhancing the quality of life for elderly individuals living in the community. Key responsibilities include:

  • Undertaking initial assessments, care planning, ongoing reviews, and monitoring of clients in accordance with MiCare's Home Care Policies and Commonwealth Government Aged Care guidelines.
  • Fostering strong relationships with internal and external stakeholders to ensure clients receive appropriate services tailored to their needs.
  • Coordinating case and planning meetings, maintaining accurate documentation, and ensuring timely follow-up on client health, care, and service delivery.
  • Developing individualised budgets for clients with Home Care Packages, adhering to spending compliance within Home Care Package Guidelines.
  • Initiating referrals and permanent placements when necessary.
  • Providing support and advocacy for clients and carers as required.
  • Promoting quality review for the programs, ensuring documentation and processes meet compliance requirements.
  • Maintaining working relationships with relevant agencies and networks to promote MiCare.
  • Responding to phone and online enquiries, engaging in public education to potential clients.

About You

  • Demonstrated expertise in sound case management within the aged care sector, preferably handling complex family and social issues.
  • Clear understanding of the Aged Care Quality Standards.
  • Solid grasp of Commonwealth Government community aged care programs (Commonwealth Home Support Program, Home Care Package program, and Veterans Home Care).
  • Proactive, reliable, excellent communication skills, and attention to detail.
  • Ability to work collaboratively and be a participative, supportive team member.
  • Capacity to meet compliance requirements, including current First Aid and CPR certification, valid Victorian driver's licence with an insured and registered vehicle, and ability to provide evidence of current flu and COVID-19 vaccination.

Qualifications and Benefits
