Job Title: Primary Health Care Nurse

The primary health care nurse plays a vital role in promoting the optimal health and well-being of children aged 0-5 years and their families through evidence-based practice.

Working collaboratively with families and a multidisciplinary team, you will assess, plan, implement, and evaluate nursing care that aligns with each child and family's goals. This includes using a strengths-based approach to partnership and primary health care principles.

Key Responsibilities:
  • Develop and implement comprehensive nursing care plans for children aged 0-5 years and their families
  • Collaborate with a multidisciplinary team to provide coordinated care that meets the unique needs of each child and family
  • Provide education and support to families on healthy lifestyle choices and disease prevention
  • Work with other healthcare professionals to ensure seamless transitions in care for children and families

To succeed in this role, you will need to be a skilled nurse with a commitment to family-centred practice and early childhood health. You should have recent experience in caring for children aged 0-5 years and their families as a registered nurse.

You will also need to be able to prioritise tasks effectively, work autonomously, and possess excellent written and verbal communication skills.
