Job Title: Transition Pathways Officer

This role focuses on supporting schools in the South East Region and Scenic Rim area to develop effective transition strategies for students at risk of not completing Year 12.

The successful candidate will work closely with school staff to provide guidance on departmental policies and procedures, as well as support the development of retention and transition plans for students. This includes identifying student needs, providing case management, and facilitating connections with employment services, community agencies, and training providers.

Key responsibilities include:

  • Supporting schools in interpreting and implementing departmental policies and procedures related to students exiting school before Year 12.
  • Collaborating with schools to develop and implement retention and transition strategies that improve outcomes for 'at risk' students.
  • Providing support to students and their families through identification of needs and intervention to facilitate successful transitions into education, training, or employment.
  • Fostering partnerships with employment services, community agencies, and training providers to create opportunities for students to engage in programs supporting successful transitions from school.
  • Maintaining effective networks with school-based support staff across a cluster of schools to ensure provision of support and interventions as necessary.
