Housing Support Professional

This position plays a crucial role in providing comprehensive support to clients in crisis.
Key Responsibilities- Conduct thorough assessments and develop tailored case plans for each client
- Regularly review and adjust case plans to ensure alignment with client progress and evolving needs
- Offer immediate crisis support and connect clients with relevant external services, promoting their overall wellbeing and stability
- Collaborate closely with real estate professionals to facilitate successful housing outcomes for clients
- Manage a portfolio of residential properties, including regular inspections and tenancy transitions to ensure smooth operations
- Maintain effective relationships with key stakeholders such as Police, Child Safety, Corrections, Youth Justice, Public Guardian, and educational institutions to deliver coordinated services
- Implement evidence-based practices to promote independent living skills, including budgeting, daily routines, and shared responsibilities, enhancing client autonomy and long-term stability
- Relevant qualifications in Mental Health, Social Work, or Community Services are highly regarded
- Proven experience working with individuals experiencing or at risk of homelessness, demonstrating empathy and understanding of complex needs
- A strong understanding of the complex needs of clients and the ability to apply this knowledge in practice
