Clinical Care Coordinator Position

We are seeking a skilled Clinical Care Coordinator to join our team. As a key member of our clinical services team, you will play a vital role in coordinating care for new and existing clients of the SA Health Community Care Program.

About The Role:

The successful applicant will be responsible for daily coordination of care, acceptance of referrals, and ensuring all client information is available to commence service delivery within required timeframes.

Key duties include identifying eligible clients for other Government funding streams and liaising with referrers, healthcare professionals, or other relevant stakeholders to facilitate smooth transfer of care.

Main Responsibilities:
  • Daily coordination of care for new and existing clients of the SA Health Community Care Program.
  • Acceptance of referrals including Metropolitan Referral Unit (MRU) and ensure all client information is available to commence service delivery within required timeframes.
  • Identify clients eligible for other Government funding streams and liaison with referrer, MRU, or healthcare professional for transfer of care.
About You:

To succeed in this role, you will need:

  • Bachelor of Nursing degree.
  • Experience in acute nursing.
  • Community nursing experience.
  • Full drivers license.
  • Previous experience managing service contracts.
  • Previous experience managing staff.
  • Strong problem-solving and time management skills.
  • Effective communication skills.
  • Computer skills.
  • Ability to work over 7-day roster.
  • Logistical experience (desirable).
