Job Description

As a Primary Care Specialist with our organization, you will be responsible for conducting virtual consultations and patient assessments, helping individuals access quality care from the comfort of their homes. Your role will involve:
  • Providing virtual consultations via phone and video for general health concerns, chronic conditions, and acute presentations
  • Reviewing structured patient histories before consultations to improve clinical efficiency
  • Carrying out follow-ups and long-term care management, ensuring continuity of care
  • Supporting a diverse patient base, including individuals in regional and remote areas who may have limited access to in-person care

Requirements

To be successful in this role, you must meet the following requirements:
  • Current AHPRA registration as a Nurse Practitioner
  • Medicare Provider Number eligibility to claim, refer, and request services
  • Proficiency with digital patient management software (telehealth experience preferred)
  • Strong interpersonal and communication skills, with a patient-centered approach
