About This Role

We are seeking a skilled Community Care Coordinator to join our team and help seniors access the support they need to maintain their independence and lifestyle choices.

This is an excellent opportunity to grow with a dedicated aged care provider in Adelaide, committed to providing exceptional care and support to their clients.

You will be working closely with experienced admin, rostering, and care staff, reporting to a supportive Home Care Manager.

  • Fantastic autonomy in the role with lots of opportunities for growth and professional development.
  • Monday - Friday role with travel involved (can be based in 3 different offices around Adelaide)
  • Work alongside other Care Coordinators in a collaborative environment that values teamwork and open communication.

The ideal candidate will have significant experience within the Aged Care industry in a similar role, with a strong understanding of the sector, including Home Care Package guidelines and service requirements.

Essential Requirements
  • Significant experience in Aged Care with a focus on Home Care Packages and a proven track record of delivering high-quality care services.
  • Excellent interpersonal and leadership skills, with the ability to communicate effectively with clients, families, and healthcare professionals.
  • Australian Drivers Licence and a reliable vehicle for travel purposes.
