Physiotherapist - Community Care Specialist

The Opportunity
We're seeking a qualified and experienced Physiotherapist to join our team as a Community Care Specialist. In this role, you'll be responsible for providing rehabilitation services to clients in their homes, as well as in residential aged care facilities.
Key Responsibilities:
- Provide rehabilitation services to clients in their homes, including rehabilitation in the home program, home care package clients, and hospital in the home program.
- Complete assessments and develop treatment plans for clients in residential aged care facilities.
- Work collaboratively with other healthcare professionals to ensure seamless care delivery.
- Travel to clients' homes and facilities, using your own vehicle (kilometre reimbursement provided).
Requirements:
- Qualifications: AHPRA registration or Limited Registration (remote level).
- Experience: A minimum of 1 year's experience in a community setting (ideally).
- Skills: Strong communication and interpersonal skills, ability to work independently and as part of a team.
