Dementia and Aged Care Services is a reputable provider of community care services in NSW, committed to delivering high-quality support for individuals and their families.
About the RoleWe are seeking an experienced Senior Care Professional to join our team as a Care Coordinator. As a key member of our staff, you will be responsible for delivering person-centred case coordination to our consumers under Consumer Directed Care Packages.
Your primary focus will be on building strong relationships with consumers and their family members, developing effective support plans, and ensuring packages are filled, waitlists are maintained, and budgets are managed effectively.
You will work closely with our Case Manager to ensure that consumer care and case coordination are performed in accordance with best practice standards, legislation, and our organisation's philosophy, policies, and procedures.
Responsibilities- To establish and maintain positive relationships with consumers and their family members, working collaboratively to develop and implement effective support plans.
- To manage and coordinate consumer care activities, ensuring timely completion of tasks and maintenance of accurate records.
- To work closely with the Case Manager to monitor consumer progress, identify areas for improvement, and make recommendations for enhanced support.
- Aged Care experience in a similar role, with a proven track record of delivering high-quality care and support.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with diverse stakeholders.
- Ability to work autonomously, prioritise tasks effectively, and manage competing demands.
- Possession of relevant qualifications and certifications, including a National Police Clearance Certificate and current First Aid & CPR Certificate.
In return for your expertise and dedication, we offer a competitive salary package, including a base salary of $70,000 per annum, plus km allowance and access to professional development opportunities.
