Job Title

A compassionate and skilled Continuing Care Worker to support individuals affected by alcohol and other drugs is sought after in this role.

The team provides holistic care, focusing on reconnection, recovery, and personal growth. The services include withdrawal management, assessment and treatment, group therapy, residential rehabilitation, and education. The trauma-informed approach empowers clients to address underlying issues and develop new skills.

This job involves providing structured before-care and aftercare, delivering individual counselling, group facilitation, AOD treatment, and case management. Collaborative work with internal teams and external services supports clients in achieving their recovery goals.

Key Responsibilities

  • Personalised care planning and wellbeing-focused support across housing, health, and social domains
  • Maintaining recovery momentum through one-to-one case management and therapeutic groups
  • Supporting clients in the Re-Entry stage of treatment, whether residing at a designated house or living independently

Requirements

Relevant VET or tertiary qualification required for the role. Significant experience supporting persons with alcohol and other drugs support needs, mental health support needs, and/or co-existing support needs is necessary. Demonstrated experience providing case management/care coordination in a similar context and environment is also required.
