Early Intervention Specialist

About Us: Alzheimer's WA provides care, support, awareness and information to assist people living with dementia, their families and carers.

We offer a range of services including advocacy, education, partnerships, training, consultancy and holistic person-centred care programs.

Our team is dedicated to providing the best possible support to individuals and families affected by dementia.

The role of the Early Intervention Specialist involves providing psychosocial support to people living with early-stage dementia and their primary support person.

This includes facilitating service navigation, performing initial eligibility and needs assessment, and delivering a short-term structured information program to the client and carer.

Key Responsibilities:

  • Conduct an initial assessment to establish eligibility and understand the needs of clients and primary support persons;
  • Provide appropriate levels of support to individuals living with early-stage dementia and their primary support persons through our Early Intervention program;
  • Offer relevant information to facilitate effective service navigation for individuals living with early-stage dementia and their primary support persons;
  • Foster peer support groups as part of our Adjusting to Change program;
  • Empower and support clients to advocate for themselves;
  • Ensure program delivery aligns with funding and contract requirements.

Requirements:

  • A degree in social work, psychology or a related field;
  • Knowledge of dementia and its impact on families and individuals;
  • A minimum of 3 years' experience in community-based services or health-related fields;
  • Ability to work independently, lead, manage multiple projects and work with high autonomy;
  • Strong written and verbal communication skills and interpersonal skills;
  • An NDIS Check (or willingness to obtain);
  • A Current WA Driver's Licence; and
  • Unrestricted working rights in Australia.
