Aged Care Specialist
">As an Aged Care Specialist, you will play a pivotal role in enhancing the lives of older adults by providing high-quality care and support. Your primary responsibility will be to conduct thorough assessments to identify the needs of this demographic and facilitate referrals to relevant services.
- Conduct comprehensive assessments to pinpoint the requirements of older adults.
- Collaborate closely with families and carers to ensure their involvement in supporting older adults.
- Ensure all assessments are conducted in accordance with Department of Health contractual requirements.
