About Our Opportunity

We are seeking a dedicated Social Worker to join our team and deliver comprehensive care to patients with varying complexity on a full-time fixed term contract.

Working in an alternate dynamic environment, you will utilise innovative technology and be rewarded in a role where your expertise will contribute to shaping exceptional health outcomes for sub-acute patients.

  • Collaborate with a multidisciplinary team to develop and implement integrated service models and clinical protocols.
  • Facilitate group sessions and contribute to health promotion activities as required.
  • Provide effective assessment and triage, timely referrals, accurate information, coordinated care and prompt follow up.
  • Advocate for patient's needs and coordinate access to services
  • Participate actively in clinical review meetings and continuous professional development.
About You

You are passionate about the provision of innovative models of care and excited by this dynamic way of working. Additionally, you have:

  • Degree in Social Work, with eligibility for membership with the Australian Association of Social Workers (AASW).
  • Demonstrated experience in a sub-acute or similar healthcare setting.
  • Strong communication, advocacy, and problem-solving skills.
  • Excellent communication and interpersonal skills, with the ability to adapt to evolving service need.
  • Proficiency in using Microsoft Office suite and other relevant documentation systems.
  • Hold a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain).
  • If successful, willing and able to provide proof of up-to-date vaccinations.
