Job Overview:

We are seeking a skilled professional to manage client goals, coordinate care plans, and develop strong relationships with clients and their families. The ideal candidate will possess excellent communication skills, be highly efficient and organized, and have a passion for delivering high-quality care services.

Key Responsibilities
  • ■Plan, manage and coordinate client goals in collaboration with multidisciplinary teams.
  • ■Communicate effectively with team members and clients to develop comprehensive care plans that cater to individual needs.
  • ■Formalise agreements with clients and their families regarding care services and associated costs.
  • ■Accurately document interactions with clients, family members or stakeholders, ensuring the sharing of relevant information is timely and secure.
  • ■Maintain regular meetings with clients and their families to review and discuss current care plans and goals.
About You
  • ■Hold a qualification in a health-related discipline, such as nursing, social work or healthcare management.
  • ■Prior experience working with seniors or aged care is essential, demonstrating your ability to provide compassionate and patient-centered care.
  • ■A minimum of one year's experience in a similar role, showcasing your expertise in care coordination and client engagement.
  • ■Demonstrate exceptional time management skills, being highly efficient and organised in a fast-paced environment.
  • ■Be able to work under pressure, adapting to shifting priorities while maintaining a high level of professionalism.
  • ■Exhibit strong written and verbal communication skills, ensuring effective collaboration with colleagues and stakeholders.
  • ■Possess a valid Australian Driver's License, allowing for flexibility in client visits and service delivery.
