Role Overview

Catholic Healthcare is seeking a compassionate and experienced Registered Nurse to join our Home & Community Team. As a Home Care Specialist, you will provide comprehensive assessment, nursing care, and evaluation of care for clients with complex health needs.

Responsibilities
  • Providing Direct Clinical Nursing Care: Deliver high-quality, patient-centered care to clients in their homes, adhering to best practice nursing procedures.
  • Assessments and Interventions: Conduct thorough clinical assessments, perform dressings, medication assessments, and catheter changing as required.
  • Nursing Care Planning: Develop holistic nursing care plans in collaboration with clients, carers, Care Advisors, and Allied Health professionals.
  • Quality Care Delivery: Provide safe, quality nursing practice to clients with specific needs or complex requirements, such as wound care, palliative care, education, and support.
  • Liaison and Collaboration: Work closely with all parties involved in client care to maintain holistic healthcare and wellbeing.
Requirements
  • AHPRA Registration: Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) following completion of a Bachelor of Nursing or equivalent qualification.
  • Post-Graduate Experience: Minimum 3 years of post-graduate nursing experience, preferably in a similar role.
  • Clinical Assessments: Proven experience in conducting clinical assessments, including PAS and RUDAS.
  • Communication and Interpersonal Skills: Strong communication skills, positive attitude, and ability to work effectively with diverse stakeholders.
  • Computer Literacy and Administrative Skills: Proficiency in Microsoft Office products and ability to adapt to new systems.
  • Transportation and Reliability: Valid driver's license and reliable vehicle for regular travel between locations.
