About the Role:

We are looking for a Registered Nurse with a strong background in paediatric care to join our team. As a Clinical Nurse, you will be responsible for case managing a portfolio of direct care clients, ensuring client-centred, individualised care, and promoting positive client outcomes.

Key Responsibilities:
  • Case manage a portfolio of direct care paediatric clients
  • Ensure client-centred, individualised care and promote positive client outcomes
  • Coordinate and support the provision of comprehensive multi-disciplinary care
  • Establish and maintain professional relationships with staff, clients, and their support network
  • Actively attract and retain health and support staff for the YNA team
Requirements:
  • AHPRA registration as a Registered Nurse
  • Relevant experience to build the capability to support complex paediatric clients living in the community
  • Exceptional communication skills and attention to detail
  • A passion for providing quality health care and community support
  • Current, unrestricted Driver's Licence
  • Current Police Clearance, WWCC, and other Clearances required
  • Ability to coordinate and lead a team (prior leadership experience preferable)
