About the Role

We are seeking a skilled and compassionate Pathways Worker to join our team at Sacred Heart Mission. As a key member of our community services team, you will play a vital role in supporting individuals who visit our Meals Program and Engagement Hub.

Key Responsibilities
  • Engage with clients to assess their needs and provide advocacy, crisis intervention, and referrals.
  • Connect clients to a range of internal and external support services, including case management and accommodation options.
  • Work collaboratively with a small team of supportive workers to provide holistic support to clients.
  • Develop and maintain relationships with community partners to enhance service delivery.
Requirements
  • Diploma or degree qualifications relevant to the delivery of community services.
  • Experience working in homelessness, mental health, and/or community settings.
  • Excellent communication skills and ability to work collaboratively as part of a team.
  • Able to work with complex clients and demonstrate de-escalation tactics towards challenging behavior.
