Job Title: Disability Care Professional

This role involves delivering high-quality care and support across various settings. You will be responsible for creating a safe and inclusive environment that promotes independence and well-being.

Key Responsibilities:

  • Deliver person-centered care and support to individuals with disabilities.
  • Assess and develop individualized care plans in collaboration with clients, families, and healthcare professionals.
  • Provide assistance with daily living tasks, education, health, careers, and other social activities.
  • Work collaboratively with multidisciplinary teams to ensure integrated service delivery.
  • Ensure accurate record-keeping and reporting of client progress and outcomes.
  • Maintain confidentiality and adhere to professional boundaries.
  • Stay up-to-date with industry developments, best practices, and regulatory requirements.
  • Promote a culture of respect, empathy, and inclusivity within the organization.

Requirements:

  • Certificate 4 in disability or relevant field (essential).
  • First aid, mental health first aid, MAPA, and medication training certificates (essential).
  • Current and valid driver's license and reliable access to a vehicle (essential).
  • Current DCSI, working with children, vulnerable people, and NDIS check (essential).
  • Current and valid police clearance (essential).
