Clinical Care Professional

We are seeking experienced and passionate professionals to deliver person-centred care across the Gold Coast South region.

About the Role:

As a Registered Nurse, you will support clients in their own homes with clinical and wellness-focused care that aligns with individual needs, preferences, and goals. You'll work with a diverse range of clients, including aged care and NDIS participants, promoting health, independence, and quality of life.

Key Responsibilities:
  • Conduct comprehensive assessments to identify client needs
  • Provide wound care, administer medication, and manage continence for optimal outcomes
  • Monitor and report on client health, ensuring accurate and timely documentation
  • Support clients with chronic disease and post-acute care requirements
  • Collaborate with care coordinators and allied health providers to deliver integrated care
  • Educate and support care staff and family members to enhance client outcomes
  • Deliver services in accordance with best practice and care standards, ensuring client satisfaction and well-being
