Optimizing Patient Care Coordination
Job Summary:The RN Case Manager plays a pivotal role in ensuring seamless care transitions from acute to post-acute settings. Collaborating with patients, families, physicians, and the interdisciplinary team, the role focuses on delivering optimal clinical outcomes and efficient resource utilization through proactive hospitalization monitoring and strategic utilization review activities.
Key Responsibilities:- Develop, implement, coordinate, and evaluate tailored patient care plans
- Lead care coordination efforts, promoting exceptional clinical outcomes
- Monitor hospitalizations and manage utilization review activities to maximize resource allocation
- Bachelor's of Science in Nursing (BSN)
- Two years' recent experience in clinical, utilization management, discharge planning, or case management roles
- High clinical competence and knowledge of disease processes and treatment protocols
- Effective communication skills with diverse healthcare stakeholders
- Ability to organize, prioritize, and problem-solve
- Demonstrated professionalism and accountability
- Understanding of patient populations' needs for dignity and respect
