Job Overview- Develops, implements, coordinates and evaluates complex patient plans targeting at-risk populations in consultation with the interdisciplinary team.
- Directs the patient through the system by coordinating activities among the care coordinator, physician, direct care staff, team members and patient.

Key Responsibilities- Comprehensively assesses the patient s needs - education, planning and resource requirements for effective, efficient and appropriate care management during an episode of illness or injury.
- Experience with Women's Health Services, including maternity, postpartum and newborn care, is advantageous.
- Bachelor's degree in Nursing (BSN).
- Graduate of a professional nursing program accredited by the Commission on Collegiate Nursing Education (CCNE) or Accreditation Commission for Education in Nursing (ACEN).
- Current license to practice registered nursing in the state.
- Five years of nursing experience relevant to the patient population, including leadership and management experience with demonstrated success.
