We are seeking a skilled and compassionate professional to facilitate our Social Support programs, providing essential services to frail, aged, or permanently disabled individuals and their carers. As a valued member of our Social Support & Wellbeing Team, you will play a vital role in designing and implementing diverse activities aimed at promoting wellness, reablement, and community participation.

Key Responsibilities:

  • Program Development: Plan and facilitate diverse activity programs tailored to meet the needs and interests of our participants.
  • Support and Advocacy: Provide advocacy, support, and information to empower individuals and their families to access relevant services and resources.
  • Collaboration and Communication: Collaborate with team members and external stakeholders to ensure holistic and coordinated care for our participants.
  • Record Keeping: Maintain accurate records and documentation in accordance with organizational policies and funding requirements.
  • Values-Based Practice: Uphold Dhelkaya Health's values of integrity, compassion, and respect in all aspects of your work

The successful candidate will have a Certificate III in Aged Care or Disability Services, demonstrated experience in supporting aged and frail or disabled individuals, ability to work both independently and collaboratively within a team, and proficiency in providing recreational and support services in a group setting. A valid driver's license is also an essential requirement for this role.

About You:

  • Experience: Experience undertaking group activity programs, especially with older adults and individuals with disabilities.
  • Skills: Strong consultation, planning, and facilitation skills to create meaningful and engaging activity programs.
  • Communication: Excellent communication skills, both verbal and written, and the ability to work collaboratively within a team

We offer a salary package of approximately $65,000 - $75,000 per annum, depending on qualifications and experience.
