We are seeking a highly skilled and compassionate Clinical Care Coordinator to join our team.
Job DescriptionThe successful applicant will be responsible for monitoring the care plans of residents, acting as an advisor in response to complex nursing and care needs, and practicing continuous improvement activities to maintain a high standard of client care.
- Monitor the care plans of residents
- Act as an advisor in response to the complex nursing and care needs of all residents
- Practice continuous improvement activities to maintain a high standard of client care
- Monitor the implementation of care plans and regularly review and update as appropriate
- Manage each resident's condition and progress, and ensure that documentation is regularly evaluated in accordance with SCC guidelines and accurately reflects ongoing care
- Liaise as required with Doctors, Paramedics, hospitals, health care professionals and other specialist service providers
To be successful in this role, you will require:
- 5 years' experience in caring for the aged or health-related area
- Registered Nurse experience
- Strong leadership skills
- Good interpersonal and written English communication skills
- Ability to play an active and positive role as part of our Care Team, providing quality care and support to frail-aged people
- Numeracy skills – reconciliations and statistics
- Sensitivity to and understanding of the needs of the elderly in a residential care setting
