Case Manager Opportunity

At our rehabilitation unit, we are seeking a dedicated and compassionate Case Manager to join our team. In this role, you will be responsible for managing client case loads and delivering high-quality care and support to residents. As a key member of our team, you will work collaboratively with colleagues to ensure the best possible outcomes for our clients.

Key Responsibilities:
  • Conduct comprehensive assessments and develop tailored treatment plans that meet individual needs.
  • Provide advocacy and support for clients who require assistance from external agencies.
  • Develop and maintain strong relationships with stakeholders to ensure seamless service delivery.
  • Identify opportunities for improvement and contribute to ongoing service development.
Requirements:

To be successful in this role, you will need:
  • A Cert IV in community services or social emotional wellbeing (or equivalent).
  • Previous experience in case management, preferably within a similar service.
  • A current Open QLD driver's license.
  • First Aid and CPR certification (or willingness to obtain).
  • Strong computer skills and ability to work with electronic systems.
  • Demonstrated knowledge of social, health, and cultural issues affecting Aboriginal and Torres Strait Islander people.
  • Commitment to workplace health and safety principles.
