Our organization seeks a skilled Nurse Practitioner to play a vital role in delivering healthcare services to individuals and communities.

Job Description:

We are looking for an experienced Nurse Practitioner who can conduct thorough adult/geriatric assessments, including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to our protocols.

  • This individual will collaborate with physicians, case managers, social workers, family members, key caregivers, and other medical personnel as required.
  • The successful candidate will work in various settings, including home visits, skilled facility visits, and other community environments.
Responsibilities:
  • Conduct comprehensive adult/geriatric assessments, including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to our guidelines and protocols.
  • Collaborate with physicians, case managers, social workers, family members, key caregivers, and other medical personnel as needed.
  • Work collaboratively with Primary Care Physicians on patient education and provide follow-up services.
  • Provide services in various venues, including home visits and skilled facility visits.

Mileage reimbursement is available. The ideal candidate will possess a Master's Degree or equivalent experience, along with satisfactory completion of NP licensure, current RN and NP licensure in the state of practice, prescription authority, Board certification by a recognized professional organization, current BLS, ACLS, or CPR certification, a minimum of 1 year experience as a Nurse Practitioner, strong computer skills, and excellent verbal and written communication skills.

Travel requirements include the ability to travel to meet patients where they live, a valid state driver's license, and proof of adequate automobile insurance coverage for the state of residence.
