We are seeking an experienced and dedicated Physiotherapist to join our team in Cowra and surrounding areas. As a Home Care Physiotherapist, you will play a vital role in delivering evidence-based physiotherapy assessment and intervention to our clients under Aged Care Funding.
This key role aims to promote independence and restore functional independence to enable clients to remain living at home. The successful candidate will work with our multidisciplinary team to provide high-quality care and support to our clients.
Key Responsibilities:
- Deliver evidence-based physiotherapy assessment and intervention to clients under Aged Care Funding
- Work collaboratively with the multidisciplinary team to provide high-quality care and support to clients
- Develop individualised programs for aged care clients within the community
- Operate effectively both as part of a multidisciplinary team and independently
- Exceptional written and verbal communication skills
- Proficient in computer literacy, with a willingness to utilise required software to facilitate remote work
Requirements:
- A Degree in Physiotherapy with current AHPRA registration
- Proven ability to assess physiotherapy needs and develop individualised programs for aged care clients within the community
- Postgraduate experience working across diverse aged care services, including community-based settings
- Thorough knowledge of health and well-being frameworks, including reablement and restorative care models
