Job Description:

Key responsibilities for this position include providing support to patients with late stage chronic and complex health conditions.

  • Develop and implement care plans tailored to the specific needs of patients.
  • Conduct palliative care needs assessments and create collaborative treatment care plans.
  • Refer patients to relevant internal and external health and community support providers as necessary.
  • Provide education and support to other clinicians on supporting complex patient needs effectively.

This role requires a high level of interpersonal and communication skills, clinical leadership capabilities, and a strong understanding of specialist palliative care and community health services.

Required Skills and Qualifications:
  • Current registration as an Occupational Therapist with AHPRA or eligibility for accreditation as an Accredited Exercise Physiologist with ESSA.
  • Extensive knowledge and experience working with consumers who have chronic and complex illnesses.
  • Experience in care planning and case management of patients in the community with chronic and complex illnesses is highly valued.
  • Understanding of specialist palliative care and community health services is essential.
  • High level interpersonal and communication skills are required for this role.
  • Clinical leadership capabilities with ability to provide consultancy, education and support to individuals, families, allied health and nursing staff, and other health professionals is desired.
