Job OverviewThe National Care Coordinator will play a vital role in ensuring the provision of high-quality clinical care to residents and participants of Cocoon SDA Care. This position requires strong leadership skills, effective communication, and a commitment to continuous improvement.
Key Responsibilities- Coordinate Care and Provide Clinical Guidance: Collaborate with Service Managers, Team Leaders, Registered Nurses, Enrolled Nurses, and Disability Support Workers to ensure seamless delivery of best practice clinical care.
- Training and Supervision: Provide coaching, mentoring, training, and supervision to support staff in managing complex care needs and maintaining high standards of care.
- Legislative Compliance: Work closely with the National Quality and Compliance Manager to ensure compliance with regulatory requirements and NDIS high-intensity skill descriptors.
- Communication and Relationship Building: Develop and maintain effective relationships with residents, participants, and support/care team members.
- Contribute to the Care/Support Team and Compliance Team: Be an active member of these teams and contribute to the development of clinical policies, procedures, and processes.
- Provide Clinical Advice: Offer expert guidance to the Care/Support Team and Compliance Team on clinical matters.
- Identify Service Gaps and Lead Continuous Improvement: Analyse service gaps and develop strategies to improve clinical practices and achieve best practice.
- Assist in Developing a Capable Care/Support Team: Coach, mentor, train, supervise, and conduct competency assessments to ensure staff are equipped to meet resident needs effectively and safely.
- Promote Choice and Independence: Foster a culture that supports participant choice and independence.
- Implement Clinical Governance Framework: Support the implementation of Cocoon SDA Care's clinical governance framework.
- Support Complex Needs and Behavioural Concerns: Manage responsive and flexible case management for residents and participants with complex needs and behavioural concerns.
- Oversight of High Intensity Support Plans: Coordinate the development and review of High Intensity Support Plans and assessments.
- Conduct Resident or Participant Re-Assessments: Conduct re-assessments, reviews, and provide ongoing support to participants, workers, and stakeholders.
- Manage Clinical Incidents and Risks: Coordinate responses to clinical incidents, risks, and complaints raised by staff, external stakeholders, and the NDIS Quality and Safeguards Commission.
- Visit Residents' Homes and Check Clinical Compliance: Conduct regular visits to residents' homes to check clinical compliance and quality.
- Prepare Participants for NDIS Review Meetings: Assist participants in preparing for their next NDIS Review Meeting, including preparation of nursing assessments/reports.
- Maintain Professional Knowledge: Stay up-to-date with professional knowledge and expertise in relation to professional skills and regulatory requirements.
- Current Unrestricted Registered Nurse Registration: Hold current registration as a Registered Nurse approved by the Australian Nurses and Midwifery Board (AHPRA).
- NDIS Worker Screening Check: Possess a valid NDIS Worker Screening Check.
- NDIS Mandatory Orientation Certificate: Have completed the NDIS Mandatory Worker Orientation course.
- Working with Children Check (WWCC): Hold a current WWCC clearance within the last five years.
- COVID-19 Infection Control Certificate: Possess a current COVID-19 Infection Control Certificate.
- COVID-19 Vaccination Certificates: Hold current COVID-19 vaccination certificates.
- First Aid, CPR, and Anaphylaxis Training: Have completed first aid, CPR, and anaphylaxis training.
- 100 Points of Identification: Provide 100 points of identification.
- Right to Work in Australia: Hold the right to work in Australia.
- Current Driver's License and Insurance: Possess a current driver's license and associated insurance.
