The RoleAs a Home Care Worker - Support Specialist, you will play a vital role in providing direct care and domestic assistance duties, as well as social support and transport as required for clients. You will work in a multidisciplinary team to ensure that client's rights, cultural, spiritual and other individual needs are incorporated in all aspects of care and daily living.
You will be responsible for:
- Providing high-quality care and support to clients in their own homes
- Working collaboratively with other healthcare professionals to ensure comprehensive care
- Identifying and addressing the needs of clients, and developing individual care plans
- Providing emotional support and companionship to clients
- Assisting clients with daily living tasks, such as bathing, dressing and meal preparation
To be successful in this role, you will need:
- A Certificate III/IV or equivalent in Individual Support (Aged or Disability) or equivalent (or currently studying a Diploma in Nursing or Bachelor of Nursing)
- Current First Aid and CPR certificates - or willing to obtain
- Current and valid Victorian Driver's Licence
- Experience in a home care setting or providing domestics support is desirable
- Knowledge in community care with a commitment to high standards of care for clients and their families
- Understanding of client directed care
- Ability to work independently and as part of a team
