This is a crucial role that provides vital support to Aboriginal and Torres Strait Islander individuals and their families by delivering respectful, culturally sensitive, and high-quality care.

Job Overview

The Community Nurse will be based across our clinics in Miami, Bilinga and Coomera. The successful candidate will have the opportunity to make a meaningful difference in the lives of our clients.

Key Responsibilities:
  • Clinical Assessments: Conduct clinical assessments in the home for aged and elderly clients in line with evidence required for funded programs.
  • In-Home Nursing Care: Provide in-home nursing care to patients including wound care, diabetic management, medication administration, stoma care and catheter and drain management.
  • Escalation: Escalate changes in patient condition to the team lead or program manager as required.
  • Clinical Notes: Maintain accurate clinical notes.
  • Interdisciplinary Collaboration: Collaborate with other care providers to ensure appropriate and timely client care.
  • End-of-Life Support: Support clients and their families through end-of-life discussions, grief, loss, and psychosocial needs.
Selection Criteria:
  • Registration: Current registration with AHPRA.
  • Nursing Qualification: Nursing qualification.
  • Community Nursing Experience: Preference of at least 1 year's experience in community nursing.
  • Culturally Safe Care: Demonstrated ability to deliver culturally safe care to Aboriginal and Torres Strait Islander clients and families.
