Key Responsibilities and ExpectationsThe Child and Youth Mental Health Service is seeking a senior nurse with experience in complex case management, assessment, counselling, and/or therapy, within mental health or relevant/related sector. You will be working within a well-supported, diverse multidisciplinary team to provide clinical coordination, assessment, and developmentally appropriate therapeutic interventions to meet the needs of young people and their families/significant others.
- Develop and implement specialist clinical coordination services that support right place, right care principles to facilitate determined need.
- Conduct high-level assessments and create individual plans that include bringing together the right mix of services across government and non-government agencies.
- Promote a positive working culture by developing effective partnerships and exceptional working relationships with all stakeholders.
- Collaborate with clinicians as part of the collaborative multidisciplinary team.
