About Us

The Transitional Aged Care Program (TACP) is a short-term service supporting older people leaving hospital. Our goal is to assist clients who may otherwise require residential aged care but who have the potential to improve their physical, cognitive, or emotional function and wish to remain at home.

This role involves working closely with a multidisciplinary team to help clients achieve their goals and safely transition from hospital back into the community. You will be well supported by our staff and collaborate with other teams to ensure a strong network around you.

We are seeking a dedicated and passionate Occupational Therapist who can deliver high-quality patient-focused care. You will bring strong interpersonal skills that enable you to work effectively with older adults in their homes, providing therapy, guidance, and support to help them improve independence and achieve their goals.

Key responsibilities of this role include:

  • Conducting home visits to assess clients' needs and develop personalized plans to improve their physical, cognitive, or emotional function.
  • Working collaboratively with other healthcare professionals to provide comprehensive care and support to clients.
  • Mentoring junior Allied Health staff to contribute to the growth and strength of our team.
