Real Community Services is seeking a Support Facilitator to enhance the lives of individuals with disabilities. If you are passionate about improving the lives of people living with a disability, we invite you to apply.

The Role:

This position involves the coordination of support in accordance with various funding bodies such as NDIA, TAC and NIISQ Participants. You will oversee the Support Staff in implementing goal-oriented support services to clients within your caseload. Your key responsibilities include case management support to participants, making referrals to services, managing a caseload and providing effective leadership to employees under your supervision.

About You:

We seek someone with demonstrated knowledge of the National Disability Insurance Scheme, organisational skills, ability to work autonomously and as part of a team, high level of interpersonal skills, professional and compassionate verbal and written communication skills, and experience supporting participants with complex medical needs.

Mandatory Requirements:

Current Blue and Yellow Card, NDIS Worker Orientation Certificate, Valid QLD driver's licence, First Aid and CPR, Flu Vaccination, Covid Vaccination including a booster, and relevant Tertiary qualifications in Social Work, Allied Health or other relevant fields are highly regarded but not essential.
