We are seeking an enthusiastic and compassionate individual to deliver person-centered care to clients in a group home setting.
Key Responsibilities:- Supporting clients to develop and maintain independence
- Advocating for clients and their individual needs
- Delivering personalized support by assisting with daily living tasks, administering medication, and manual handling
- Transporting clients using wheelchair accessible vehicles and community access
- Monitoring the well-being of clients and reporting on their progress
As a Disability Support Practitioner, you will work collaboratively with our team to provide high-quality care and support services to our clients. You will have the opportunity to make a real difference in the lives of others and contribute to a supportive and inclusive work environment.
