Locum Social Work Opportunity

We are seeking an experienced Social Worker to join our team on a locum basis. This role will involve caseload coverage, collaboration with external providers, and liaison with family supports.

  • Caseload includes Transitional Care Program, Rehab in the Home, and GEM Program.
  • Collaborate with external providers and My Aged Care system.
  • Liaise with family supports, explore long-term accommodation options, and address social issues.
Key Requirements
  • ASAP start for 8 weeks with possible extension.
  • Knowledge of Victorian and NSW Aged Care systems preferred.
  • A driving licence is required.
  • 5 days per week, Monday to Friday.
