Lead Care Coordinator Opportunity

We are seeking an experienced leader with a strong background in the Disability sector, particularly in complex behaviors and mental health challenges. As the Lead Care Coordinator, you will play a pivotal role in providing exceptional support to our clients with both physical and psycho-social disabilities.

Your key responsibilities will include:

  • Overseeing daily operations and coordinating individualized care plans.
  • Fostering a collaborative environment that empowers both team members and our clients.

This role requires hands-on support to our clients and availability Monday to Friday and weekends when required.

You will be responsible for supporting the financial performance of the site through effective management of operating expenditure. Additionally, you will ensure team member awareness of each individual's requirements and provide support aligned to the person's plans, goals, and any other requirements.

We require a dedicated professional who can deliver accurate documentation of all activities and contribute to creating an inclusive culture that fosters employee growth, learning, and optimal performance.
