About the Role

We're looking for a Care Coordinator to support carers—family, friends, and significant others—of people who have experienced a suicidal crisis. Based in Maroochydore, this is a non-clinical, community-based role where you'll provide tailored psychosocial support and facilitate peer-led engagement. This is a fixed-term role until 30 June 2026, delivered as part of the Integrated Mental Health Program and funded by the Country to Coast PHN.

What You'll Be Doing

This role focuses on meaningful, person-centred engagement. You'll be the first point of contact for many carers, helping them feel heard, understood, and supported.
  • Conduct intake assessments and develop personalised support plans
  • Facilitate peer-led one-on-one and group support sessions, in-person or via telehealth
  • Deliver structured workshops to equip carers with practical tools and knowledge
  • Coordinate referrals across health, community, and social services
  • Monitor carer engagement, escalate safety concerns, and contribute to program reporting and improvement
What You'll Bring

Your lived experience and understanding of the carer journey are central to this role. We're looking for someone with a calm, empathetic presence and the ability to guide others through complex systems with care and confidence.
  • Lived experience supporting a loved one through suicidal crisis
  • Relevant qualifications (e.g. Social Work, Psychology, Counselling, Mental Health) or equivalent experience
  • Experience in care coordination, peer support, or carer-focused services
  • A strong understanding of trauma-informed, recovery-oriented, and culturally responsive practice
  • Confidence using digital tools and systems, and working both independently and as part of a team
