About the Role

We are seeking a highly skilled and experienced Service Coordinator to join our team at InLife Independent Living Ltd. As a Service Coordinator, you will play a critical role in providing exceptional support to our clients with complex needs.

Key Responsibilities
  • Lead a team of disability support workers to provide high-quality services to clients
  • Develop and maintain positive relationships with clients, their families, and other stakeholders
  • Work collaboratively with our Client Service Managers to ensure seamless service delivery
  • Provide ongoing support, mentoring, and training to staff members
  • Contribute to the continuous improvement of our policies and procedures
  • Participate in our on-call roster and work on shifts with clients
Requirements
  • Successful track record in disability, health, or community services, including experience in direct client support and team coordination
  • Passion for client-focused care and a collaborative approach
  • Active listening skills and ability to build meaningful relationships
  • Tech-savvy and comfortable using cloud-based apps for rostering and incident reporting
  • Well-developed communication skills and ability to engage professionally with staff, clients, and their families
  • Relevant qualifications and/or experience in Community Services/Case Management
  • Full Victorian driver's licence and suitable vehicle
