  • Conduct visits to assess client needs and develop plans for therapy and support
  • Perform assessments and treatments for physical and neurological conditions
  • Assess home environments and recommend aids, equipment, and modifications as required
  • Support clients in their homes to promote self-care development and reablement
  • Evaluate client mobility and control in various environments
