Advanced Practice Nursing Role

Job Summary:

We are seeking a highly skilled Clinical Nurse Specialist to provide comprehensive primary health and emergency care in a culturally secure environment.

This role involves working as part of a multidisciplinary team within a Primary Health Care framework, partnering with the community and other organisations to achieve optimal patient outcomes.

Key Responsibilities:

• Assess patients' needs and develop individualised care plans
• Provide high-quality nursing care and education to patients and their families
• Collaborate with other healthcare professionals to ensure effective communication and coordination of care
• Participate in quality improvement initiatives and contribute to a culture of safety and excellence
• Maintain effective relationships with patients, families, carers, and other stakeholders

Requirements:

• Current registration as a Registered Nurse with the relevant professional body
• Postgraduate qualification in a relevant field (e.g., nursing management, education, or research)
• A minimum of 5 years' experience in a clinical setting, preferably in primary health care
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Knowledge of legislative and regulatory requirements related to nursing practice
