Care Coordinator for Residential Aged Care

We are seeking a highly motivated and compassionate Care Coordinator to join our team at Calvary. As a key member of our Residential Aged Care team, you will play a vital role in providing exceptional care and support to our residents.
Key Responsibilities- Care Planning: Work with residents, families, and healthcare professionals to develop and implement personalized care plans that cater to their unique needs and goals.
- Activity Coordination: Coordinate and deliver engaging activities and therapies that promote social interaction, cognitive stimulation, and emotional well-being.
- Documentation and Communication: Accurately document resident progress, maintain up-to-date care plans, and communicate effectively with stakeholders to ensure seamless care delivery.
- Collaboration and Teamwork: Work closely with multidisciplinary teams to ensure comprehensive care, share knowledge, and provide support as needed.
- Certificate III or IV in Leisure & Lifestyle: You hold a recognized qualification in this field, or equivalent experience in Aged Care is highly desirable.
- Experience in Activity Planning and Coordination: You have a proven track record in planning, implementing, and evaluating activities and programs for frail individuals with cognitive impairment and other disabilities.
- Excellent Communication and Interpersonal Skills: You possess excellent written and verbal communication skills, with the ability to build strong relationships with residents, families, and colleagues.
- Reliability and Flexibility: You are available to commit to required shifts, including evenings and weekends, and are flexible to meet the changing needs of our residents.
