Job Title: Clinical Navigator

Our organization seeks an experienced and innovative individual to work in the Transition Care Program (TCP).

The TCP is a program designed to assist frail aged patients reach their maximum functioning capacity and determine their long-term care arrangements.

The TCP operates in both community and residential settings, offering opportunities to enhance experience along the care continuum.

Key Responsibilities:

  • Provide case management services to patients with complex psychosocial needs.
  • Work collaboratively with multidisciplinary teams to develop and implement patient care plans.
  • Assess and prioritize patient needs, developing strategies to improve patient outcomes.
  • Support clinicians within the TCP program as required.

Requirements:

  • Eligibility/Membership of relevant professional body.
  • Experience in sub-acute and/or community setting.
  • Demonstrated competence in a specific area of professional practice.
  • Capacity to supervise students.
  • Knowledge of/experience in Aged/Residential Care settings.
  • Current Driver's Licence.
  • Demonstrated participation in Professional Development.
  • Interest and/or experience in Quality improvement and research activities.
  • Computer literate.
