Collaborate with Vulnerable Populations
Key Responsibilities
Conduct comprehensive health assessments and develop clinical and psychosocial interventions.
Work collaboratively on case planning, attend multidisciplinary meetings, and advocate for clients with medical and support services.
Support client transitions from residential services to community living, ensuring continuity of care.
Maintain accurate client records, case notes, and reports in line with professional standards.
Provide medical care, administer medication, and facilitate referrals to health services within a recovery framework.
Current registration with the relevant healthcare authority.
Minimum of 3 years' full-time experience in community-based nursing, mental health, or alcohol and drug sectors.
Proven ability to develop and implement effective care plans, manage complex health needs, and work collaboratively as part of a multidisciplinary team.
Experience working with Aboriginal and Torres Strait Islander communities and an understanding of cultural considerations.
Familiarity with trauma-informed care principles when working with vulnerable populations.
Current driver's license and valid Working with Children check (or ability to obtain one).
